Compassionate Care
Sunrise Adult Foster Care (AFC) Program is a MassHealth funded program that offers a cost-effective alternative to long-term care placement. Our program allows MassHealth recipients who require assistance with activities of daily living (ADLs) to age in place with a friend or relative (caregiver).
Our Mission
Community First
As an AFC provider, we ensure the client and caregiver meet all the necessary qualifications defined in the Massachusetts Regulation. We conduct required background checks (CORI, SORI, OIG) for caregivers periodically to ensure safety and compliance. Our dedicated team ensures the client is safe and receives the required care through monthly visits by a Registered Nurse and an assigned case manager.
